Samuel was born in Derby, on the 31st March, 1894. He enlisted in Ottawa, Ontario, 17th November 1915, stating his occupation as a farmer, and his next of kin as his mother, Kate Beardsley, of 4, Wright Street, Derby.
He was five feet two and a half inches in height, had a medium complexion, brown eyes and brown hair, and he was a Presbyterian. He had the scar from a herniaectomy on his right side.
